Application for Form (Application for Respite Care or Permanent Entry to an Aged Care Home)
Barrabill House is for those who are no longer able to live at home and need ongoing help with every day tasks or health care assistance.
When you have decided to place your name on the waiting list at Barrabill House complete the Application Form and you or your representative will be contacted once a placement becomes available for you.
Permanent Residential Aged Care Request for a Combined Assets and Income Assessment
The Department of Human Services or the Department of Veterans’ Affairs requires information about your combined assets and income for permanent residential care purposes.
The information that you provide is used to calculate your daily care fees and charges.
You can choose not to provide your income and asset details and not complete the assessment form however, you may be asked to pay the maximum applicable fees as guided by the Department of Human Services.
